Position Overview

The Housing Support Case Manager is responsible for facilitating essential services that empower individuals to achieve personal and economic independence. This role involves assisting clients in developing tailored housing plans and ensuring they reside in well-maintained living environments.

Key Responsibilities

1. **Program Compliance**: Adhere to program regulations and ensure clients receive high-quality services that meet established standards.
2. **Client Admissions**: Screen potential participants and facilitate their entry into the program, reviewing essential documents and agreements.
3. **Independent Housing Plans**: Collaborate with clients to create individualized housing plans that address their specific needs and goals.
4. **Resource Networking**: Develop and maintain a network of resources for referrals, ensuring clients have access to necessary services.
5. **Collaboration**: Work alongside various service providers to support clients in fulfilling their housing plans and program requirements.
6. **Direct Services**: Provide clients with assistance in budgeting, daily living skills, and utility management as needed.
7. **Client Meetings**: Conduct regular face-to-face meetings with clients to monitor progress and address any concerns.
8. **Property Inspections**: Regularly inspect living spaces to assess compliance with program standards and address any maintenance needs.
9. **Documentation**: Complete all required data entry and reporting in a timely manner, ensuring all client files are accurate and up-to-date.
10. **Conflict Resolution**: Mediate issues between clients and property management regarding rental payments and community living standards.

Qualifications

- **Education**: A Bachelor’s Degree is required; a Master’s Degree in human services or a related field is preferred.
- **Experience**: Minimum of two years in a related field such as mental health, housing, or social services.
- **Communication Skills**: Strong verbal and written communication abilities are essential; proficiency in English is required, and bilingual skills (Spanish) are a plus.
- **Technical Skills**: Basic proficiency in Microsoft Office applications and internet navigation.
- **Analytical Skills**: Ability to create simple financial budgets and prioritize tasks effectively.
- **Physical Requirements**: Capability to conduct home visits, including walking and climbing stairs.
- **Interpersonal Skills**: Ability to engage with underserved populations and work collaboratively within a team environment.
