Position Overview

The role of the Housing Support Case Manager is crucial in assisting program participants to achieve personal and economic self-sufficiency. This position focuses on ensuring that individuals are equipped with the necessary skills to secure, maintain, and thrive in permanent housing.

Key Responsibilities

  • Ensure compliance with program regulations and provide high-quality services to clients.
  • Conduct screenings for program admissions and facilitate the onboarding process.
  • Assist clients in formulating Independent Housing Plans (IHP) tailored to their specific housing needs and aspirations.
  • Offer a range of support services including information, referrals, counseling, and advocacy.
  • Develop and maintain a comprehensive resource network to address the diverse needs of clients.
  • Conduct regular face-to-face meetings with clients to monitor progress and provide support.
  • Perform inspections of living spaces to ensure compliance with program standards and assist families in maintaining their housing.
  • Document and report on the physical needs of housing units and common areas.
  • Lead educational and recreational group sessions to foster community engagement.
  • Complete all necessary data entry and reporting in a timely manner to maintain accurate records.
  • Facilitate communication between clients and property management regarding rental agreements and housing-related issues.
  • Prepare discharge plans and manage the exit process for clients transitioning out of the program.

Qualifications

  • A Bachelor's Degree is required; a Master's Degree in a relevant field is preferred.
  • Minimum of two years of experience in a related field such as mental health, housing, or social services.
  • Strong verbal and written communication skills; proficiency in English is essential, and bilingual skills are advantageous.
  • Basic proficiency in Microsoft Office applications and internet navigation.
  • Ability to develop simple financial budgets and manage basic math tasks.
  • Physical capability to conduct home visits and navigate various environments.
  • Strong organizational skills and the ability to work independently or collaboratively.
