Position Overview

The Housing Support Case Manager plays a crucial role in facilitating personal and economic independence for program participants. This position is dedicated to ensuring that individuals have the necessary resources and support to secure and maintain quality housing.

Key Responsibilities

  • Ensure compliance with program regulations and provide high-quality services to clients.
  • Conduct assessments and facilitate the admission process for new participants.
  • Assist clients in creating Independent Housing Plans (IHP) tailored to their unique needs and goals.
  • Offer information, referrals, and advocacy services to support clients in their housing journey.
  • Build and maintain a network of resources for effective referrals and collaboration with various service providers.
  • Conduct regular face-to-face meetings with clients to monitor progress and provide support.
  • Inspect living conditions to ensure compliance with program standards and assist families in maintaining their housing.
  • Document and report on client progress and housing unit needs in a timely manner.
  • Lead group sessions focused on social and psychoeducational development.
  • Manage administrative tasks related to client files and program documentation.

Qualifications

  • A Bachelor's Degree is required; a Master's Degree in a related field is preferred.
  • Two years of experience in fields such as mental health, homelessness, or housing services.
  • Strong verbal and written communication skills; proficiency in English is essential, and bilingual abilities are a plus.
  • Basic proficiency in Microsoft Office applications.
  • Ability to develop simple financial budgets and make home visits as needed.
  • Strong organizational skills and the ability to work independently or collaboratively.
