Job Summary

RNnetwork is seeking a skilled travel nurse RN Case Manager to join our team in a dynamic and challenging role.

Key Responsibilities
  • Provide high-quality patient care and case management services in an acute care setting.
  • Collaborate with healthcare teams to develop and implement effective care plans.
  • Conduct thorough assessments and evaluations to identify patient needs and develop strategies for optimal care.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Manage and coordinate patient care services, including referrals, authorizations, and discharge planning.
  • Stay up-to-date with industry developments and best practices in case management and patient care.
Requirements
  • Current RN licensure and certification in case management.
  • Minimum 2 years of experience in acute care case management.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong analytical and problem-solving skills.
