Job Summary

As a Social Worker at LifeBridge Health, you will play a vital role in ensuring that our patients receive comprehensive, family-oriented care in the comfort of their own homes. Your expertise will be instrumental in assessing patients' and families' psychosocial, environmental, and financial needs, and developing personalized plans of care that address their unique challenges.

Key Responsibilities
  • Conduct thorough assessments of patients' and families' needs, identifying areas for improvement and developing targeted interventions
  • Collaborate with patients, families, and other healthcare professionals to develop and implement comprehensive care plans
  • Provide counseling services to patients and families, offering emotional support and guidance throughout the dying and grieving process
  • Coordinate with community and agency homecare referrals to ensure seamless delivery of services
  • Maintain accurate and detailed documentation of patient interactions and care plans
Requirements
  • Master's Degree in Social Work from an accredited institution
  • Minimum of three years of clinical experience, with a preference for experience in community health settings
  • Current licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) in Maryland and Pennsylvania
