Job Summary

As a Social Worker at LifeBridge Health, you will play a vital role in ensuring that all services are delivered in accordance with our Mission Statement and S.P.I.R.I.T. Values. You will provide professional, comprehensive, family-oriented services to individuals in the home setting, assessing patients' and families' psychosocial, environmental, and financial needs. You will formulate, implement, and evaluate a plan of care in collaboration with patient, family, and other caregivers, and provide case management as appropriate. You will also offer counseling services to provide information and support during all facets of the dying and grieving process to help the hospice patient/family cope with terminal illness. Additionally, you will coordinate with community and agency homecare referrals for services.

Responsibilities

  • Assess patients' and families' psychosocial, environmental, and financial needs.
  • Formulate, implement, and evaluate a plan of care in collaboration with patient, family, and other caregivers.
  • Provide case management as appropriate.
  • Offer counseling services to provide information and support during all facets of the dying and grieving process.
  • Coordinate with community and agency homecare referrals for services.

Requirements

  • Master's Degree in Social Work from an accredited school.
  • Minimum of three years of clinical experience; one year in a community health setting preferred.
  • LCSW - Certified Social Worker.
  • LCPC - Licensed Clinical Professional Counselor.
