Job Summary
  • Deliver professional, comprehensive, and family-oriented services to individuals in the home setting.
  • Assess patients' and families' psychosocial, environmental, and financial needs.
  • Formulate, implement, and evaluate a plan of care in collaboration with patient, family, and other caregivers.
  • Provide counseling services to support patients and families during the dying and grieving process.
  • Coordinate with community and agency homecare referrals for services.
Key Responsibilities
  • Develop and implement personalized care plans to meet the unique needs of patients and families.
  • Conduct thorough assessments of patients' and families' needs, identifying areas for improvement and developing strategies for resolution.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Provide emotional support and counseling to patients and families, promoting emotional well-being and resilience.
  • Stay up-to-date on industry developments and best practices, applying knowledge to improve patient outcomes and care delivery.
Requirements
  • Master's Degree in Social Work from an accredited school.
  • Minimum of three years of clinical experience, with one year in a community health setting preferred.
  • LCSW or LCPC licensure required in Maryland and Pennsylvania.
