Nurse Practitioner Hospice PRN

As a Hospice Nurse PRN, you will provide direct nursing care to patients in accordance with established policies, procedures, and protocols of the healthcare organization.
Key Responsibilities:
- Implement and monitor patient care plans, ensuring timely and effective care delivery.
- Coordinate with interdisciplinary teams to provide comprehensive patient care.
- Assess and coordinate patient discharge planning needs with healthcare team members.
- Perform point-of-care testing as needed and maintain accurate records.
- Perform other duties as assigned by management.
Requirements:
- Graduate of an accredited school of nursing.
- No experience required, but prior experience in hospice care is preferred.
Work Environment:
- Constant standing and walking.
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, reaching, and keyboard use/data entry.
- Occasional lifting/moving of patients.
- Occasional bending, stooping, kneeling, squatting, twisting, gripping, and repetitive foot/leg and hand/arm movements.
