**Job Description:**

Senior Community Care RN Role Summary: As a Senior Community Care Registered Nurse with Optum, you will have the opportunity to work in a fast-paced environment with a dynamic team to deliver high-quality care to our members in their homes. Your primary responsibilities will include collaborating with an interdisciplinary care team, providing education and support to members, and participating in care coordination activities.

Main Responsibilities:

  • Communicate effectively with members, families, and healthcare providers to ensure seamless care transitions.
  • Collaborate with the interdisciplinary care team to develop and implement effective care plans.
  • Ensure accurate and complete documentation of member interactions and care plans.
  • Participate in facility-based meetings and contribute to the development of policies and procedures.

Requirements:

  • Education: Associate degree or higher in Nursing (RN)
  • Licensure: Active and unrestricted license in the state where you reside, or ability to obtain by start date
  • Certifications: Basic Life Support certification
  • Skill Requirements: Ability to complete physical requirements, including lifting a 30-pound bag; ability to perform physical assessment
  • Travel Requirements: Ability to travel between facilities
