Job Summary:

We are seeking a skilled Obstetrical Field RN Care Coordinator to join our team at UnitedHealth Group. As a key member of our healthcare team, you will be responsible for engaging with members to complete comprehensive needs assessments, identifying medical, behavioral, functional, cultural, and socioeconomic needs.

Key Responsibilities:

  • Conduct face-to-face and/or telephonic assessments to gather information about members' needs and develop personalized care plans.
  • Collaborate with healthcare providers, community resources, and other stakeholders to ensure members receive comprehensive care.
  • Develop and implement care plans that address medical, behavioral, functional, cultural, and socioeconomic needs.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.

Requirements:

  • Current RN licensure in the state of practice.
  • Experience in obstetrical care and case management.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
