Care Navigator Specialist

Capital City Hospice
The patient-centered approach of Capital City Hospice empowers our team to maximize resources at bedside, ensuring that patients receive the highest quality care in a community-based setting.
We prioritize providing employees with the tools they need to excel in their roles, resulting in exceptional hospice care in Dayton, OH. With manageable caseloads and flexible quotas, our team can devote more time to each patient.
This role will collaborate with our Hospice Liaison team and Admissions team to drive admissions for patient placement. As the first point of contact for hospitals, healthcare facilities, and other referral sources, you'll establish relationships through networking opportunities and education initiatives. You'll identify appropriate care and services for patients and families according to attending physician and plan of care, as well as company policy and procedures.
A comprehensive benefits package includes health, dental, vision, life insurance, disability, 401k with match, paid holidays, and 17 days off annually.
Key Responsibilities:
- Candidate Requirements:
- Bachelor's degree in marketing, business administration, or related field preferred; acceptable combination of experience and/or training may be considered instead
- Licensed registered nurse in the state of practice is a plus
- Possess ability to make independent decisions when circumstances warrant
- Possess ability to deal tactfully with patients, family members, visitors, agency personnel, and general public
- Knowledgeable of quality assessment and assurance procedures
- Valid driver's license and proof of insurance required
Estimated Salary: $65,000 - $85,000 per year
