**About the Role:**

The Ohio State University Wexner Medical Center seeks a skilled Patient Care Navigator to facilitate seamless transitions for patients through the healthcare system.

**Job Summary:**

As a key member of our interdisciplinary team, you will work closely with physicians, nurses, and other healthcare professionals to provide comprehensive care management assessments, treatment plans, and follow-up evaluations for patients. Your primary focus will be on efficient, effective, and compassionate inpatient care, ensuring a safe transition to outpatient settings.

**Responsibilities:**

• Monitor patient care processes and physiological needs, documenting and communicating data to evaluate and improve timeliness and appropriateness of care.

• Collaborate with healthcare teams to provide disease-specific assessment, treatment, and follow-up evaluation for patients.

• Stay up-to-date with state and national healthcare trends, proactively responding with initiatives to enhance care provision and ensure efficient resource utilization.

**Requirements:**

• Bachelor's degree in Nursing (Master's preferred)

• RN licensure

• Five years of recent clinical experience applicable to the population (preferred)

• Knowledge of evidenced-based practice and disease-specific processes

• Technology support and payer protocols desired
