Job Description

At Trinity Health Corporation, we are seeking a compassionate and skilled Home Health Hospice Care Navigator to join our team. In this role, you will provide care in an atmosphere sensitive to each person's physical, emotional, social, and spiritual needs.

About the Job

The Home Health Hospice Care Navigator will be responsible for assessing and evaluating patient care for an assigned group of patients, utilizing evidence-based practice. You will collaborate with patients and families to develop and implement a plan of care that includes physical, spiritual, and educational components.

Responsibilities
  • Nursing Process: Assess and evaluate patient care for an assigned group of patients, utilizing evidence-based practice.
  • Collaborate with patients and families to develop and implement a plan of care that includes physical, spiritual, and educational components.
  • Accurately communicate pertinent information to appropriate team members in a timely manner.
  • Maintain current knowledge, skills, education, and licensure in clinical practice.
Requirements
  • Minimum 2 years' work experience as a professional nurse.
  • Ability to operate a motor vehicle required; possesses a valid driver's license from the State of Ohio and has access to a functional automobile during work hours.
  • Criminal background check and fingerprinting completed as required by Ohio law.
  • Current license to practice in the state of Ohio.
