Key Responsibilities:

  1. Collaborate with multidisciplinary teams to coordinate clinical programs and services for designated patient populations.
  2. Develop and implement strategies to improve transplant outcomes and patient satisfaction.
  3. Ensure compliance with regulatory requirements and maintain accurate records.
  4. Communicate effectively with patients, families, and healthcare teams to provide excellent patient care.
  5. Participate in quality improvement initiatives and contribute to a culture of excellence within the organization.

Requirements:

  • Bachelor's degree in Nursing or related field is required.
  • Masters in Nursing or related field and specialty certification are preferred.
  • Minimum 3 years of experience in transplant coordination or related field.
  • New York State Nurse/NP/PA License with current registration.

Compensation Package:$125,000 - $155,000 per year
