About the Role:

At Mount Sinai Hospital, we are seeking an exceptional Renal Transplant Program Coordinator to join our esteemed team of healthcare professionals. This role is a crucial part of our mission to provide unparalleled patient care and drive medical excellence.

Job Summary:

The Renal Transplant Program Coordinator will be responsible for coordinating the daily activities of the Renal Transplant Team, ensuring seamless communication and collaboration among all stakeholders. This individual will work closely with patients, families, surgeons, physicians, and other healthcare professionals to provide comprehensive care and support throughout the transplant journey.

Responsibilities:

  • Coordinate multidisciplinary screening and evaluation of patients for renal transplant, including lab tests, medical clearance, and waiting list management.
  • Facilitate organ offers and coordinate timely communication with patients, surgeons, and other team members.
  • Collaborate with social workers, dietitians, pharmacists, and finance teams to ensure comprehensive patient support.
  • Maintain accurate records and databases, ensuring compliance with regulatory requirements and hospital policies.
  • Participate in continuous quality improvement initiatives and contribute to the development of best practices within the program.

Requirements:

  • Bachelor's degree in Nursing or related field.
  • Minimum 2 years of experience in acute care nursing, preferably in transplant or critical care settings.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks simultaneously.
  • Familiarity with UNOS and transplant regulations.
