Medical Transplant Program Coordinator

The Medical Transplant Program Coordinator role at the Mount Sinai Health System is an exciting opportunity for a skilled and compassionate nurse to join our team. This position is responsible for coordinating clinical programs and services for designated patient populations within the Transplantation Institute.
About the RoleThis role will work closely with transplant surgeons, hepatologists, nephrologists, social work, nutrition, pharmacy, and finance teams to ensure seamless care coordination. The ideal candidate will have excellent communication and organizational skills, with the ability to prioritize tasks effectively.
Key Responsibilities- Coordinate multidisciplinary screening and evaluation of patients
- Ensure all necessary lab tests are performed on potential transplant candidates
- Communicate with patients regarding waiting list status
- Foster collaboration between healthcare teams to optimize patient outcomes
- NYS RN with BSN and BCLS
- At least 2 years of acute care nursing experience
- Critical care experience and/or specific experience with patient population preferred
The estimated salary for this role is $120,000 - $150,000 per year, depending on experience.
