Job Title: Lead Care Manager ECM Program

Job Summary:

The Lead Care Manager ECM Program is responsible for coordinating and implementing organization-wide enhanced Care Management. This role works closely with medical, dental, behavioral health, and vision teams and patients to develop personalized care plans that address the unique needs of each patient.

Key Responsibilities:

  • Oversee and implement provision of Enhanced Care Management services, including identification and achievement of Care Plan goals and objectives with the member.
  • Advocate on behalf of members with healthcare professionals and connect with ECM members via phone or in-person to facilitate engagement, assessment, follow-up, and education/training visits.
  • Use motivational interviewing and trauma-informed care practices to work with ECM members to identify Care Plan goals and objectives.
  • Coordinate pre-operative evaluations and linkages to outside health education, wellness programs, and community resources.
  • Assist with translation for staff when necessary and utilize SCFHC's NextGen and other electronic tracking systems to coordinate services and input data for reporting.
  • Review existing SCFHC protocols and policies to match ECM requirements and outreach to patients who have missed preventative services.

Requirements:

  • High School Diploma or equivalent required.
  • Bachelor's Degree preferred.
  • Medical Assistant/LVN certification preferred.
  • Knowledge of community resources in the area.
  • Minimum of 2 years' experience in case management in a community clinic or FQHC setting.
  • Comfortable working with diverse populations and bilingual in English/Spanish (oral and written).
  • Ability to work flexible hours.
