Job Title: Enhanced Care Management Registered Nurse Care Team Manager

AIDS Healthcare Foundation is seeking a highly skilled and compassionate Enhanced Care Management Registered Nurse Care Team Manager to join our team. As a key member of our care coordination team, you will play a vital role in ensuring that our health plan members receive the highest quality care and support.

Key Responsibilities:

  • Develop and implement personalized care plans for health plan members
  • Cultivate and facilitate care coordination with interdisciplinary teams
  • Close care gaps and engage members in opportunities to increase knowledge of government-funded programs, health plan benefits, and community resources
  • Lead and centralize the interdisciplinary team to ensure members receive appropriate, individualized, and enhanced healthcare

Requirements:

  • CA RN License required
  • BSN preferred
  • Acute care experience preferred
  • Care Management experience preferred
  • Managed Care experience preferred
  • Bilingual Spanish preferred
