Job Summary

UPMC is seeking a skilled Nurse Clinical Care Coordinator to join our team. As a key member of our interdisciplinary team, you will be responsible for delivering high-quality nursing care and coordination to our patients.

Responsibilities
  • Conduct comprehensive face-to-face assessments to identify members' medical, behavioral, and social needs.
  • Develop and implement individualized care plans to address gaps in care and promote healthy lifestyles.
  • Collaborate with members, caregivers, and providers to achieve desired outcomes and improve health outcomes.
  • Provide education and support to members to enhance their knowledge and skills related to health and lifestyle management.
  • Coordinate and modify care plans with members, caregivers, PCP, specialists, community resources, and other health plan and system departments as appropriate.
Requirements
  • Current licensure as a Registered Nurse (RN) in the state where the facility is located or a multistate license issued by a participating NLC state.
  • Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
  • Driver's License.
  • One year of health insurance experience preferred.
  • One year of experience in clinical, utilization management, home care, discharge planning, and/or case management preferred.
