Job Description

The Clinical Care Coordinator at Ascend will play a vital role in monitoring patient symptoms through technology and telehealth visits. With excellent attention to detail, the coordinator will monitor and document symptoms, health data, and current medical conditions. The successful candidate will be able to provide consistency of care with patients and work closely with our in-house clinicians to ensure positive outcomes.

  • Collaborate with our clinical team on follow-up plans for patients being monitored between visits using wireless monitoring devices and mobile apps.
  • Document and review patient charts with accurate diagnoses and current conditions, as well as responses to treatments.
  • Communicate regularly with patients regarding concerns, current conditions, and responses to treatments.
  • Monitor wireless device and app data for changes in responses and health outcomes.
  • Escalate critical issues to clinicians regarding patient health.
  • Document all interactions with patients in our company portal.

Requirements:

  • At least 2 years of experience in primary care, family practice, geriatrics, and disease management.
  • A current LVN license and associate's or bachelor's degree preferred. Coursework approved by the California Nursing Board is highly valued.
  • An ideal candidate will have good working knowledge of Medicare, chronic condition management, remote patient monitoring, and quality measurements.
  • Excellent listening skills and highly professional communication abilities are essential for discerning concerns through regular communication.
  • Strong technology skills, including MS Office, Google, and EMR systems.
  • A highly organized and proactive individual who can juggle administrative and clinical duties.
  • Able to think quickly, resolve problems professionally, and possess a proactive and organized work style.
  • Familiarity with preventative physical examinations consistent with HCC and Annual Wellness Visit criteria is desirable.
  • Previous customer service or sales and marketing experience is highly valued.

Compensation:

  • $24 - $27 per hour.
