About the Job:

As a RN Care Manager at HexaQuEST Health, Inc., you will have the opportunity to work in a dynamic and supportive environment where you can make a meaningful impact on patients' lives. You will collaborate with a multidisciplinary team to assess, plan, implement, and evaluate patient care plans.

Responsibilities:
  1. Patient Evaluation: Conduct comprehensive evaluations of patients to identify factors contributing to their disease process.
  2. Care Coordination: Coordinate care plans with healthcare teams to ensure continuity of care.
  3. Discharge Planning: Develop and implement discharge plans to ensure smooth transitions from hospital to home or other healthcare settings.
  4. Medical Necessity: Determine medical necessity for patient care services and communicate with third-party payers as necessary.
  5. Clinical Leadership: Provide leadership to members of the healthcare team relative to discharge and care planning.
Requirements:
  • Certification: Current NH RN License
  • Education: Associate's degree in Nursing from an accredited nursing program; Bachelor's degree in Nursing preferred
  • Experience: 2 years of Acute Care Facility experience in a clinical specialty or Care Management, Discharge planning, Quality Improvement, and Utilization Review
Compensation:

$80,000 - $105,000 per year, based on experience and qualifications
