Job Summary

We are seeking a skilled RN Transitional Care Coordinator to join our team at Interim HealthCare. As a key member of our sales team, you will be responsible for marketing our home care services to potential clients and referral sources, and for developing and maintaining relationships with healthcare professionals and organizations.

Key Responsibilities
  • Consultative Selling: Assess potential clients' needs and provide solutions that align with their unique care requirements.
  • Lead Generation & Conversion: Identify and qualify new leads through outreach and referrals, and convert leads into new clients by providing exceptional service and tailored solutions.
  • Collaboration: Work closely with clinical, operational and sales teams to establish internal referral processes and create personalized care packages.
  • Relationship Management: Establish and maintain contact and relationships with referral sources, including physicians, hospitals, skilled and other nursing facilities, community resources and others.
  • Follow-Up: Conduct regular follow-up with existing clients to ensure satisfaction, address concerns, and offer additional services when necessary.
  • Sales Reporting: Track and report on activity, and monitor account development status and plan next steps.
  • In conjunction with the VP of Strategic Development, establish and monitor the process to track all referral activity, including sources and disposition.
  • Develop and work a quarterly business plan for key accounts established and modified in conjunction with the VP of Strategic Development.
Requirements
  • RN with Hospice experience strongly preferred.
  • 2-4 years of experience in hospice or healthcare sales.
  • Strong understanding of hospice and personal care services.
  • Demonstrated effective written and verbal communication skills, including the ability to communicate with all levels within and outside the company.
  • Demonstrated familiarity with home healthcare services, referral sources and payors.
  • Computer and Internet literacy.
  • Demonstrated excellent organizational and detail skills.
  • Demonstrated ability to accomplish goals under short deadlines, changing priorities and minimal resources.
  • Proven ability to work independently with minimal supervision.
