About the Role

We are seeking a skilled Palliative Care Navigator to join our team at Agape Care Group. As a Palliative Care Navigator, you will play a critical role in providing high-quality, patient-centered care to individuals with complex healthcare needs.

Key Responsibilities
  • Coordinate patient care across the healthcare continuum, ensuring seamless transitions and optimal outcomes.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Identify and address health system barriers, improving access to quality care for patients and families.
  • Function as a clinical advocate and educator, providing support and guidance to patients and families.
  • Build and maintain relationships with community partners to ensure reliable access to quality care.
Requirements
  • Current state license as a LPN or MSW (Social Worker preferred).
  • Minimum 5 years of experience in a healthcare setting, with at least 3 years in Hospice and/or Palliative Care.
  • Reliable transportation and ability to sit, stand, bend, move intermittently, and lift at least 25lbs.
