Job SummaryWe are seeking a highly skilled Ambulatory Clinical Pharmacist, Infusion to join our team at the Hospital of the University of Pennsylvania (HUP) at the Perelman Center for Advanced Medicine (PCAM). This full-time, day shift position will be responsible for preparing and dispensing medications, providing drug information, and ensuring continuity of pharmaceutical care.
Key Responsibilities- Prepare and dispense medications using appropriate techniques to ensure optimal patient outcomes.
- Provide timely and accurate responses to drug information requests from healthcare providers and patients.
- Ensure seamless continuity of pharmaceutical care between acute and ambulatory settings.
- Participate in the assessment, management, and reporting of adverse drug events.
- Manage medical emergencies and adhere to medication-use policies and guidelines.
- Manage time effectively to fulfill practice responsibilities and comply with accreditation, legal, regulatory, and safety requirements.
- Current pharmacist license in the state of Pennsylvania.
- Bachelor of Arts or Science or PharmD from an accredited College of Pharmacy or Equivalent.
- Previous oncology and infusion experience preferred.
