Bilingual Care Manager II, BSN/LICSW

AmeriHealth Caritas is seeking a highly skilled Care Manager II, Registered Nurse/Licensed Independent Clinical Social Worker (RN/LICSW) to join our team in the Washington, DC region. This is a unique opportunity to make a meaningful impact in the lives of our members and contribute to the success of our organization.
Key Responsibilities- Assess members with complex medical needs to determine care coordination and case management requirements.
- Develop and implement individualized care plans to address members' physical, behavioral health, and social needs.
- Coordinate care and services with healthcare providers, community resources, and other stakeholders to ensure seamless care delivery.
- Provide medication management, including regular medication reconciliation and support for medication adherence.
- Identify and implement interventions to address members' issues and promote optimal health outcomes.
- Document progress and outcomes in a timely and accurate manner.
- Current and unrestricted RN/LICSW licensure in the District of Columbia.
- Minimum of 3 years of clinical practice experience as a RN or LICSW working with adults and/or pediatrics with complex medical and behavioral health diagnoses.
- Minimum of 3 years of case management experience supporting Medicaid members within a managed care organization.
- Ability to work in a hybrid environment with associates working from home two days a week and in the office three days a week.
- Must reside in the DC Metro area.
- Bilingual (Spanish/English) required.
- A competitive salary range of $82,400 - $112,300.
