Job Title: Supportive Living Program Coordinator

Join the State of Illinois Department of Healthcare and Family Services as a Supportive Living Program Coordinator. This role is responsible for managing the Home and Community Based Services (HCBS) waiver-Supportive Living Program (SLP).

About the Role

The ideal candidate will have a passion for social service and leadership qualities that ensure the future success of this program. As a Supportive Living Program Coordinator, you will be responsible for:

  • Planning, directing, and evaluating the Department's HCBS Waiver-SLP
  • Directing reviews of complaints, incident reports, and annual review results of HCBS Waiver-SLP providers
  • Developing and implementing procedures to monitor and control transmittal of HCBS Waiver Supportive Living Program data
  • Consulting with SLP providers to ensure access for HCBS Waiver-SLP services
  • Developing procedures and establishing controls to ensure compliance with Quality Assurance standards for the HCBS waivers-SLP
Requirements

To be successful in this role, you will need:

  • Knowledge, skills, and mental development equivalent to completion of four (4) years of college, preferably with courses in business or public administration
  • Prior experience equivalent to three (3) years of progressively responsible administrative experience in a public or business organization
Preferred Qualifications

The ideal candidate will have:

  • Two (2) years of experience working with Home and Community Based Services (HCBS) waiver(s)
  • One (1) year of experience regarding the waiver requirements, policies, rules and regulations surrounding the program and/or HCBS waivers, including but not limited to performing actions in the CMS 1915(c) Waiver Applications and 372 Reports portal
