Job Summary

Family Service and Guidance Center Topeka is seeking a skilled School Based Case Manager to join our team. As a School Based Case Manager, you will work collaboratively with school districts to create a system of care for children and youth with serious emotional disturbance (SED).

Key Responsibilities
  • Provide direct services to children and youth with SED and their families based on the strengths model.
  • Work with school districts to create a system of care that promotes good mental health and academic performance.
  • Facilitate outreach to students, families, and child welfare contacts to engage in treatment.
  • Participate in treatment planning and maintain treatment plans and protocols.
  • Communicate with school personnel about student needs, interventions, and progress.
  • Provide individual supportive counseling, solution-focused interventions, and emotional and behavioral management.
  • Engage with students in the classroom, home, or community to build skills.
  • Make referrals to community resources and promote access to needed services.
  • Assist with crisis planning and act as an advocate for children and youth.

Requirements
  • BA/BS degree in a human service field or 4 years of education/experience in a human service field.
  • Valid driver's license, current auto insurance, and a good driving record.
  • Ability to pass background investigations.
  • Experience working with children and families preferred.

Working Environment
The School Based Case Manager will work in an office and community-based setting with a flexible schedule. The position requires the ability to deal with individuals in crisis and may involve exposure to aggressive and/or acting out children and youth.
