Job SummaryThe Operational Supervisor Care Coordination RN is responsible for overseeing the care coordination process, ensuring seamless transitions of care, and providing guidance to care coordinators. This role requires strong leadership skills, excellent communication, and a deep understanding of healthcare operations.
Key Responsibilities- Manage caseloads and supervise care coordination staff to ensure effective bed management and efficient care delivery.
- Collaborate with care coordinators to resolve complex cases and provide guidance throughout the shift.
- Communicate and coordinate with the House Coordinator to identify care coordination actions needed to ensure effective bed management.
- Work with Manager, Care Coordination Leadership, and other campuses as needed to ensure alignment and effective care delivery.
- Nursing degree and three (3) years of previous experience as a hospital care coordinator or insurance case manager.
- Current Standards of Care & Criteria, working knowledge of Intequal and/or Milliman.
- Ability to train and direct employees.
- Bachelor's degree in nursing.
- Five years of experience in care coordination or a related field.
- Leadership experience preferred.
- Certification as a Certified Case Manager (CCM) or Accredited Case Manager (ACM).
