Hospice LPN Care Specialist

About the RoleWe're seeking a skilled and compassionate LPN to join our team. As a hospice care professional, you'll work closely with our interdisciplinary team to provide high-quality care to patients in their homes, nursing homes, or assisted living facilities. Your primary responsibilities will include:
- Providing direct patient care, including administering medications, wound care, and other treatments
- Implementing individualized care plans, taking into account the patient's unique needs and preferences
- Collaborating with the healthcare team to ensure seamless care coordination
- Providing education and support to patients and their families on disease management, symptom control, and end-of-life care
- Participating in ongoing quality improvement initiatives to enhance patient outcomes
