Dynamic Case Manager for LGBTQ Housing Support

The San Diego LGBT Community Center seeks a skilled and compassionate Recovery Focused Case Manager to join our team in supporting vulnerable youth at the Sunburst Youth Housing Project. This is a full-time, regular non-exempt position.

Job Summary:

This role involves providing case planning, substance use recovery support, individualized supportive services, conducting assessments, organizing community-building events, facilitating workshops, and crisis support to participants of the Sunburst Youth Housing Project. The Recovery Focused Case Manager will work collaboratively within a two-case manager team to help program participants achieve housing stability and self-sufficiency.

Responsibilities:
  • Conduct intakes, assessments, and evaluations with clients to identify their unique needs and goals.
  • Provide links to mental health services, substance use recovery services, facilitate access to detox and treatment programs responsive to each participant's needs.
  • Collaborate with internal and external providers to support client care plans and ensure comprehensive service delivery.
  • Organize community-building events and facilitate workshops that promote client well-being and self-sufficiency.
Requirements:
  • Education: Bachelor's degree in a social service-related field and/or experience working in the field of case management.
  • Language Skills: Fluency in English required.
  • Computer/Office Equipment Skills: Must be comfortable working in PC/Windows-based environments and have strong proficiency with Microsoft Office.
  • Other Qualifications:
  • Demonstrated ability to manage multiple tasks with shifting priorities.
  • Must demonstrate competence in working with diverse populations and using strength-based, trauma-informed, and Motivational Interviewing Techniques.

Work Location: On-site, in the office and with program participants in the field. The office is located in the East Village Neighborhood in the City of San Diego.

Compensation & Benefits:

The salary range for this position is $24.00/hour - $26.00/hour.
