About the Role: In this role, you will provide community-based health education services to underserved populations. The ideal candidate will have experience in chronic disease management programs and be fluent in English and Spanish. You will also develop and implement diabetes self-management education programs and participate in multidisciplinary team meetings.

Earn an Estimated $63,500 per year as you work to improve the health outcomes of our community members. Your responsibilities will include analyzing and interpreting general business periodicals, professional journals, technical procedures, or governmental regulations and preparing and disseminating educational materials.
