About the Role

We are seeking a skilled and compassionate LPN to join our team as a Home Health Care Specialist. As a key member of our care team, you will provide high-quality nursing care to patients in their homes, helping them to recover from illness, injury, or surgery.

Key Responsibilities
  • Provide professional nursing services under the direction of a RN, in compliance with state and federal regulations, and CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitation, recovery, and prevention.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Document patient progress, outcomes, and any changes in their condition, maintaining accurate and detailed records.
  • Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid drivers license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
