Patient Care Facilitator

First Choice Health Centers is seeking a dedicated Care Coordinator to collaborate with our Behavioral Health team. This role is essential in delivering comprehensive and coordinated care through effective teamwork, case discussions, and outreach initiatives.
Work Schedule:
The work hours for this position are structured as follows: Monday, Wednesday, Thursday from 8:00 am to 4:30 pm; Tuesday from 8:00 am to 6:00 pm; and Friday from 8:00 am to 3:00 pm.

- The hourly compensation for this role is $19.50/hour, influenced by various factors including experience and certifications.

Essential Qualifications:
- CPR Certification required
- Bilingual proficiency in English and Spanish is preferred
Experience and Education:
- At least an Associate's degree with two years of experience in care coordination, health navigation, or community health education (equivalent experience may substitute for education with a minimum of a high school diploma)
- Understanding of the Social Determinants of Health
- Must possess reliable transportation for onsite services at various locations
Reporting Structure:
- Reports directly to the Director of Behavioral Health
Key Responsibilities:
- Facilitate referrals to appropriate community and state services and resources
- Educate patients regarding First Choice Health Center's Behavioral Health Services, Internal Medicine, Family Medicine, Dental, and other specialty providers
- Participate in Community Programs to foster referral relationships and patient engagement
- Maintain updated patient records in the designated electronic medical record system related to care coordination activities
- Conduct timely outreach to patients referred to the Care Coordination Program and effectively manage patient caseloads
- Adhere to all patient safety protocols and infection control guidelines
