Job Opportunity

Eminence Home Care is seeking a dedicated and compassionate Caregiver/ Direct Care Worker to join our team in the Greater Hartford County, Middlesex and Tolland counties in Connecticut.

As a Home Health Caregiver, you will provide direct care and support to patients in their homes or in a healthcare facility, ensuring they receive the highest level of care possible. You will play a crucial role in helping patients live their best lives.

Key Responsibilities:

  • Assist patients with daily living activities such as bathing, dressing, and grooming.
  • Monitor vital signs and record patient information.
  • Administer medications as directed by healthcare professionals.
  • Provide emotional support to patients and their families.
  • Collaborate with healthcare professionals to develop and implement care plans.
  • Ensure a safe and clean environment for patients.

Requirements:

  • Prior experience with dementia patients and senior care.
  • High School diploma or equivalent.
  • Access to reliable transportation.
  • Excellent communication and interpersonal skills.
  • Capable of handling heavy lifting and meeting the physical demands of the role.
  • Strong attention to detail.
