Key Responsibilities:
- Conduct comprehensive initial assessments for patients in case management.
- Offer expert consultation to the interdisciplinary care team.
- Deliver therapeutic interventions to patients and their families.
- Implement effective case management strategies to overcome current and future barriers to care.
- Facilitate referrals to necessary support services.
- Initiate the development of discharge plans in collaboration with patients.
- Organize family and treatment team meetings early in the admission process, as clinically appropriate.
- Engage in supervision and team meetings to enhance collaborative practice.
- Adhere to the ethical standards and scope of practice as outlined by the National Association of Social Workers.
Qualifications:
- Master of Social Work (MSW) required.
- Licensed Independent Clinical Social Worker (LICSW) preferred.
- Minimum of 2 years of supervised clinical experience required.
- Proven ability to establish rapport with patients, families, and community partners.
- Strong verbal and written communication skills.
- Expertise in treatment and crisis intervention.
- Familiarity with community resources and support systems.
