Job Summary:

The Physical Therapist is a qualified professional who directs, supervises, evaluates, and provides therapy services toward a rehabilitative plan of care to patients in the home health setting.

Responsibilities:
  • Create and update individualized patient care plans to address physical therapy needs.
  • Evaluate patients by accurate diagnosis and prognosis, and treating patients to alleviate pain, improve function and enhance their ability to perform at maximum potential.
  • Create and maintain patient records and clinical notes in a timely manner.
  • Attend and participate in case conferences with local teams monthly, and actively participate in staff development opportunities.
  • Oversee Physical Therapy Assistants in accordance with Executive Council for Physical Therapy and Occupational Therapy Examiners.
  • Communicate between agency, clients and your Area Support Manager to ensure well-coordinated approach to patient care.

Requirements:
  • Bachelor's degree in a physical therapy related field.
  • Doctor of Physical Therapy degree from an American Physical Therapy Association (APTA) accredited program.
  • Active Texas licensure or certification as a Physical Therapist.
