About the Role
The Care Manager is responsible for providing individualized care and services to a group of residents, ensuring their physical, emotional, and social needs are met. You will work closely with the care team to develop and implement personalized care plans, which focus on enhancing the quality of life for our residents. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities
- Provide direct care and support to residents, including assistance with activities of daily living
- Develop and implement individualized care plans that meet the unique needs of each resident
- Collaborate with the care team to ensure seamless delivery of care and services
- Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards

Requirements
To be successful in this role, you will need:
- High school diploma or equivalent required; certified nursing assistant (CNA) or certified medical assistant (CMA) certification preferred
- Prior experience working in healthcare or senior care setting
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
