About the Job Description Summary

As a Healthcare Partnership Specialist, your key responsibilities will include collaborating with interdisciplinary staff and stakeholders to promote patients' optimal health through disease prevention and management strategies. You will also implement evidence-based interventions to improve patient outcomes, coordinate care services, and analyze data to identify areas for improvement.

Job Requirements and Qualifications
  1. Bachelor's Degree in a related field (Required)
  2. RN licensure in Ohio (Required)
  3. CPR certification (Required)
  4. Valid driver's license (Required)
  5. Care Management experience (Preferred)
