**Job Summary**

The Care Coordination Specialist will be responsible for managing transitions between settings, caregivers, and providers for hospice and home-based palliative care patients. This role requires strong communication and collaboration skills, as well as the ability to facilitate seamless transitions.

To excel in this role, you should have a BSN degree, current licensure as a registered nurse in Ohio, and valid driver's license. Experience in pediatric care and certifications such as Pediatric certification or Advanced-Continuity of Care Certification (A-CCC) are preferred.
