Job Title: Patient Care Management Coordinator

We are seeking a highly skilled Patient Care Management Coordinator to join our team at Fairview Health Services.

Job Summary:

The Patient Care Management Coordinator provides comprehensive care coordination of patients as assigned, assessing their plan of care and developing, implementing, and monitoring the utilization of resources and progress of the patient through their care.

Key Responsibilities:
  • Facilitate weekend discharges and perform patient assessments
  • Manage patients across the healthcare continuum to achieve efficient clinical, financial, operational, and satisfaction outcomes
  • Act as a single point of contact for patients, physicians, and care providers throughout the patient's hospitalization
  • Initiate and implement transition functions and activities for patients, communicating with patients, families, and the healthcare team to ensure seamless transitions
  • Assess and document learning needs of the patient and family, developing an individualized education plan
  • Establish and implement nursing care plans for assigned patient populations through assessment, development, activation, evaluation, and discharge
  • Collaborate with healthcare team partners and patients/family to manage the patient discharge plan
  • Optimally communicate the plan across the continuum of care
  • Assist in the development and implementation of process improvement activities to achieve efficient clinical, financial, and satisfaction outcomes
  • Collect data and other information required by payers to fulfill utilization and regulatory requirements
  • Identify and communicate issues related to case escalation to the appropriate leader
  • Establish a collaborative relationship with physicians, medical directors, nurses, and other unit staff, and payers
  • Demonstrate effective communication by being a critical link with attending and consulting physicians and all healthcare team members and payers
  • Facilitate resolution to any identified issues
  • Mentor internal members of the healthcare team on case management and managed care concepts
  • Understand and focus on key performance indicators
  • Actively supervise outcomes and participate in quality planning
  • Facilitate integration of concepts into daily practice
Qualifications:
  • AD in Nursing
  • 5 years clinical experience
  • 1+ years working as a care coordinator/case manager
  • Active MN Registered Nurse license
  • Preferred: Case Management Certification, BSN, Basic Life Support (BLS)
