Job Title: Patient Care Coordinator

Job Summary:

We are seeking a detail-oriented and dedicated Patient Care Coordinator to join our team at Home Health Care, Inc. As a Patient Care Coordinator, you will play a crucial role in providing a seamless patient experience and ensuring all documentation and information is collected and organized appropriately.

Key Responsibilities:

  • Send and track required face-to-face documentation for Physicians to sign and return
  • Collect information for currently hospitalized clients: H&P (History & Physical), ROC (Resumption of Care), MD orders, etc.
  • Conduct pre-admit calls to verify client information, as well as five-day and discharge follow-up calls
  • Track hospitalized clients and notify Community Liaisons of discharges, in order to resume providing care

Requirements:

  • High school graduate; college degree preferred
  • Knowledge of medical terminology is strongly preferred
  • Minimum of one (1) year of experience in an office or health care setting preferred
  • Strong verbal, written, and telephonic communication skills
  • Demonstrated ability to type at least 50 words per minute
