Occupational Therapist Home Care Specialist

We are seeking an Occupational Therapist to join our team in providing high-quality home care services. The successful candidate will work under the supervision of a Care Manager to provide therapy to assigned patients and contribute to their overall well-being.
About the RoleThis per diem position includes benefits such as career development, whole person wellbeing resources, and mental health resources and support. As an Occupational Therapist, you will be responsible for assessing, planning, implementing, and evaluating total patient care, ensuring compliance with therapy standards and regulations.
Key Responsibilities- Conduct thorough evaluations to assess patient function and identify areas for improvement.
- Develop and implement effective treatment plans to help patients achieve specific outcomes.
- Maintain accurate records of patient progress and communicate regularly with healthcare teams.
- Collaborate with occupational therapists and other healthcare professionals to ensure seamless care coordination.
- Masters degree in Occupational Therapy from an accredited institution.
- Current Occupational Therapy License in the State of Practice.
- Minimum six-months relevant occupational therapy experience.
- Recent experience in a Medicare-certified home health agency as an Occupational Therapist.
