Overview

The Afterhours AIM RN will report directly to the Director of AIM.

This role involves identifying and coordinating the needs of patients and families while delivering preventive and supportive care to those facing palliative conditions.

Utilizing assessment, intervention, and educational skills, you will maximize patient comfort and improve quality of life.

You will assume afterhours coverage for designated teams, functioning as an interdisciplinary advanced illness management support, with telephonic, video, and in-person interventions during afterhours.


Key Responsibilities:

  • Triage patient needs over the phone until an in-person visit can be arranged
  • Respond promptly and effectively to crisis calls from patients and families after hours
  • Conduct initial nursing evaluations to determine eligibility for palliative services within 48 hours of referral
  • Assess and reassess the physical, psychosocial, emotional, and environmental needs of patients and families
  • Collaborate with patients, families, attending physicians, and interdisciplinary team members to provide comprehensive care
  • Document problems, goals, interventions, and patient/family responses to palliative care in a timely manner
  • Maintain accurate clinical notes regarding patient conditions
  • Communicate effectively with AIM team members, clinical supervisors, and patients' primary care providers regarding changes in patient status
  • Report patient conditions and identified needs/issues to supervisors or nurse practitioners
  • Oversee the utilization of medications, durable medical equipment, and supplies in accordance with patient needs and physician orders
Qualifications:

Education:

  • Associate's or Bachelor's degree in Nursing

Experience:

  • Minimum of 2 years of experience as a Registered Nurse
  • Proven ability to deliver quality patient care
  • Preferred experience in palliative care, oncology, hospice, or geriatric nursing
  • Additional experience in hospital settings, emergency care, critical care, or similar areas is advantageous

Licenses and Certifications:

  • Current RN license to practice in the state
  • CPR Certification

Skills:

  • Proficient in Microsoft Outlook, Word, and Excel
  • Experienced in electronic medical record documentation
  • Strong compassionate communication skills
  • Independent and sound critical decision-making abilities in patient care delivery
  • Competent in performing afterhours in-person visits
  • Familiar with the needs of patients and families facing advanced illnesses
  • Excellent written and verbal communication skills
  • Commitment to maintaining patient confidentiality
  • Strong problem-solving and time management skills
