About the Role

We are seeking a highly skilled Palliative Care Coordinator Registered Nurse to join our team at Medical City Plano. As a Palliative Care Nurse Coordinator, you will play a vital role in providing individualized goal-directed nursing care that promotes, advocates for, and strives to protect the health, safety, and rights of our patients and their families.

Responsibilities
  • Plan and provide individualized nursing care to patients with complex medical needs
  • Collaborate with interdisciplinary teams to develop and implement patient care plans
  • Provide education and support to patients and their families on palliative care and end-of-life care
  • Participate in quality improvement initiatives to enhance patient care and outcomes
  • Develop and maintain strong relationships with patients, families, and healthcare teams
Requirements
  • Texas State RN license or compact license required
  • BSN requirement
  • National Specialty Certification (CNRN, CCRN)
  • BLS and ACLS certification required
  • Minimum of two (2) years experience working in neuroscience/trauma
  • Exceptional organization and communication skills
  • High interest in Stroke, Neurovascular, TBI and Trauma
  • Must practice great degree of autonomy and professional initiative
