Job SummaryWe are seeking a skilled Registered Nurse to join our team as a Palliative Care Nurse. The successful candidate will work with patients, their families, and other healthcare professionals to provide and coordinate high-quality end-of-life care. The ideal candidate will have prior experience in palliative care, oncology, hospice, or geriatric care, and will be able to provide quality patient care and education to patients and families.
Key Responsibilities- Conduct in-person and telephonic assessments to identify patient support needs and develop care plans.
- Provide education to patients and families on disease process, self-care techniques, end-of-life care, medications, nutrition, and dietary needs.
- Communicate with patients, families, and healthcare professionals to ensure seamless care coordination.
- Document timely clinical records on all patients, including screening/assessment findings, physician orders, progress notes, and care plans.
- Collaborate with the interdisciplinary team to develop and modify care plans as needed.
- Registered Nurse license to practice in the state.
- 2 years of prior skilled Registered Nurse experience.
- Demonstrated ability to provide quality patient care and education to patients and families.
- Fluent competence in computer programs Microsoft Outlook, Microsoft Word, and Microsoft Excel.
