Job Summary

We are seeking a highly motivated and compassionate Case Manager to join our team at Beach 21 Supportive Housing. As a Case Manager, you will work closely with tenants to identify and address their needs, provide support and guidance, and help them achieve their goals.

Responsibilities
  • Maintain professional relationships with clients and ensure confidentiality
  • Practice Universal Precautions and comply with all relevant policies and procedures
  • Review documentation and create and maintain tenant files
  • Conduct initial intake and assessment of tenants and their families' needs
  • Develop and implement independent living plans and provide support and guidance to tenants
  • Assist tenants in accessing community resources and making referrals to appropriate services
  • Monitor tenants' progress and document via progress notes
  • Participate in case conferences and follow up with tenants and referral organizations
  • Provide required information for reports and may recommend closing of cases
Requirements
  • Bachelor's degree and experience working with the Seriously and Persistently Mentally Ill (SPMI) population
  • Ability to maintain fingerprint clearance and comply with all relevant policies and procedures
  • Flexibility to work occasional evening shifts
