Job SummaryWe are seeking a skilled Case Manager to join our team at Beach 21 Supportive Housing. As a Case Manager, you will work closely with tenants to identify and address their needs, develop personalized plans, and connect them with community resources. This is a full-time position that requires a strong understanding of social services, excellent communication skills, and a passion for supporting vulnerable populations.
Key Responsibilities- Maintain professional relationships with clients and ensure confidentiality.
- Practice Universal Precautions and Standard Protocol & Procedures.
- Comply with Federal, State, City, and CAMBA security and privacy policies.
- Review documentation and create tenant files.
- Conduct initial intake and assessment of tenants' needs and periodic re-assessments.
- Develop and implement independent living plans with tenants.
- Connect tenants with community resources and services.
- Monitor tenants' progress and document via progress notes.
- Participate in case conferences and follow up with tenants and referral organizations.
- Bachelor's degree in a related field and experience working with SPMI populations.
- Ability to maintain fingerprint clearance and comply with Mandated Reporter requirements.
- Flexible hours may be required occasionally.
