Job Summary

We are seeking a skilled Case Manager to join our team at Beach 21 Supportive Housing. As a Case Manager, you will work closely with tenants to identify and address their needs, develop personalized plans, and connect them with community resources. This is a full-time position that requires a strong understanding of social services, excellent communication skills, and a passion for supporting vulnerable populations.

Key Responsibilities
  • Maintain professional relationships with clients and ensure confidentiality.
  • Practice Universal Precautions and Standard Protocol & Procedures.
  • Comply with Federal, State, City, and CAMBA security and privacy policies.
  • Review documentation and create tenant files.
  • Conduct initial intake and assessment of tenants' needs and periodic re-assessments.
  • Develop and implement independent living plans with tenants.
  • Connect tenants with community resources and services.
  • Monitor tenants' progress and document via progress notes.
  • Participate in case conferences and follow up with tenants and referral organizations.
Requirements
  • Bachelor's degree in a related field and experience working with SPMI populations.
  • Ability to maintain fingerprint clearance and comply with Mandated Reporter requirements.
  • Flexible hours may be required occasionally.
