Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Lake Health District. As a Clinical Pharmacist, you will be responsible for providing safe, effective, and prompt medication therapy to our patients.

Key Responsibilities
  • Assess patient needs and develop individualized pharmaceutical care plans
  • Provide direct patient care in coordination with physicians, nurses, and other hospital personnel
  • Participate in all aspects of the medication-use system, including quality management and regulatory compliance initiatives
  • Conduct patient care quality and regulatory compliance initiatives to improve medication-use processes and pharmacy practice
  • Provide medication and practice-related education/training to staff
Requirements
  • Graduate of an ACPE accredited College of Pharmacy
  • Licensed (or eligible for licensure) in the state of Oregon
  • Clinical pharmacy experience in a hospital setting preferred
