Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Lake Health District. As a Clinical Pharmacist, you will be responsible for providing safe, effective, and prompt medication therapy to our patients.
Key Responsibilities- Provide comprehensive and individualized pharmaceutical care to patients
- Assess patient needs and incorporate age and disease-specific characteristics into drug therapy and patient education
- Adjust care according to patient response and provide clinical interventions to detect, mitigate, and prevent medication adverse events
- Participate in quality management programs to improve medication-use processes and pharmacy practice
- Provide medication and practice-related education/training to staff
- Graduate of an ACPE-accredited College of Pharmacy
- Licensed (or eligible for licensure) in the state of Oregon
- Clinical pharmacy experience in a hospital setting preferred
