Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Lake Health District. As a key member of our healthcare team, you will be responsible for providing safe, effective, and prompt medication therapy to our patients.
Key Responsibilities- Provide comprehensive and individualized pharmaceutical care to patients with a broad range of disease states.
- Assess patient needs, incorporate age and disease-specific characteristics into drug therapy, and provide clinical interventions to detect, mitigate, and prevent medication adverse events.
- Participate in all necessary aspects of the medication-use system, including quality management and regulatory compliance initiatives.
- Collaborate with physicians, nurses, and other hospital personnel to deliver optimal medication therapy and support medication-use systems.
- Develop and implement medication therapy plans, monitor patient progress, and adjust care as needed.
- Graduate of an ACPE-accredited College of Pharmacy.
- Licensed (or eligible for licensure) in the state of Oregon.
- Clinical pharmacy experience in a hospital setting preferred.
